Summer School 2020If you are having trouble scanning the QR code on the summer school flyer, please click the link below to register for summer school. The form will be available to fill out on Thursday, 5/21 at 6:30am.
When schools are closed to the public, the Certificate of Enrollment required for a student's DDS appointment can be ordered by completing our request form and will be available for pick up every Friday from 8:00 a.m. - 4:00 p.m. at our Central Office located at 405 N. Ash Street, Springfield, GA 31329. There is a $5.00 fee for each copy. You may pay online with Visa, MasterCard, and Discover or you may pay at the time of pickup with cash, money order, or check. CLICK HERE to submit your request.
Any Requests received after 12 p.m. on Friday will be available for pickup the following Friday.
CERTIFICATE OF ENROLLMENT REQUESTSIf your student needs a certificate of enrollment for their DDS appointment, please COMPLETE THIS FORM by Thursday at NOON each week.
You will be asked to pay online or when you pick up. The link for paying online will be on the second page of the form.
ADAP certificate- (Health class alcohol and drug awareness program)
Include your full name, date of birth and put ADAP in the subject.
