Athletic Physicals

  • Print out the forms below and bring them to your appointment.  
  • The doctor's office will give your GHSA Physical Examination Form and GHSA Medical Eligibility Form (2 sheets signed by the doctor) back to you to take home.
  • Student athletes must have a parent or guardian with them during their appointment.
  • All physicals will be by individual appointments only. There will be NO team physical day.
