Physical Information:

A current physical and medical eligibility form must be uploaded as well as all athletic forms completed electronically in Dragonfly before a student is allowed to participate in any tryout, work out, practice or athletic event.   

SEMS will be using the DRAGONFLY MAX app. Athletic forms must be completed electronically and uploaded on DRAGONFLY before a student can participate in any Athletics. 

• The physical exam forms and medical eligibility, which must be completed by a physician, may be printed below. These forms must be uploaded directly to the DRAGONFLY MAX app. 
• All other forms can be completed electronically in the DRAGONFLY MAX app

PLEASE NOTE: DRAGONFLY is BEST navigated from a Desktop, Laptop or Chrome style computer.
